Pharmacology
- Mechanism of action: Inhaled agents produce general anesthesia through central depression; local anesthetics block sodium channels to stop nerve conduction in a region.
- Onset / peak / duration: Inhaled rapid and titratable; local anesthetics minutes to onset with duration by agent (bupivacaine long).
- Half-life / therapeutic level: Variable; no routine level.
Clinical use
- Indications: General anesthesia (inhaled), local and regional anesthesia and nerve blocks (local), lidocaine also as an antiarrhythmic and topical.
- Usual dose, route, frequency: Inhaled by anesthesia provider; local by injection, topical, or patch.
- Maximum dose / adjustments: Respect maximum local anesthetic doses (especially bupivacaine, which is cardiotoxic); epinephrine-containing solutions not used in end-arterial areas (fingers, toes, nose, ears).
Safety
- Contraindications: Malignant hyperthermia history (volatile agents are triggers), amide anesthetic allergy.
- Black box warning: None for these prototypes.
- Interactions: Other CNS depressants; local anesthetics with antiarrhythmics (additive cardiac effects).
- Pregnancy / lactation: Used in anesthesia care as needed.
- High-alert: No.
Adverse effects
- Common side effects: Nausea after general anesthesia; local numbness and tingling.
- Serious effects to report: Malignant hyperthermia (volatile agents), local anesthetic systemic toxicity (CNS excitation then seizures, cardiac toxicity), bupivacaine cardiotoxicity.
- Antidote / reversal: Dantrolene for malignant hyperthermia; lipid emulsion for local anesthetic systemic toxicity.
Nursing process
- Assessment before administration: Malignant hyperthermia history, allergies, vital signs.
- Interventions during therapy: Continuous monitoring; have dantrolene available in surgical areas; aspirate before injecting local anesthetic.
- Monitor: Temperature and rigidity (malignant hyperthermia), CNS and cardiac signs.
- Evaluation / expected outcome: Adequate anesthesia.
Patient teaching
- Patient teaching: Report any family history of anesthesia reactions before surgery.
- Notify provider if: Not applicable (monitored setting).
- Administration tips: Monitored settings; respect local anesthetic dose limits.
