Pharmacology
- Mechanism of action: Activates both GIP and GLP-1 receptors for strong glucose-lowering and appetite-reducing effects.
- Onset / peak / duration: Weekly SubQ; effect builds over titration.
- Half-life / therapeutic level: Half-life about 5 days; no routine level.
Clinical use
- Indications: Type 2 diabetes (Mounjaro), chronic weight management (Zepbound).
- Usual dose, route, frequency: SubQ 2.5 mg weekly, titrated up to 15 mg weekly.
- Maximum dose / adjustments: Titrate every 4 weeks for tolerance.
Safety
- Contraindications: Medullary thyroid carcinoma history, MEN 2.
- Black box warning: Thyroid C-cell tumor risk (rodents).
- Interactions: Delayed gastric emptying, insulin and secretagogues; may reduce oral contraceptive effectiveness (counsel on backup method).
- Pregnancy / lactation: Discontinue.

Adverse effects
- Common side effects: Nausea, diarrhea, decreased appetite, constipation.
