Verse 1: I-CHECK & Interview

Bare skin exam, I verbalize clear,

Pulse for thirty, times it by two,

Respirations same thing I do.

Blood pressure cuff, temp oral read,

Pulse ox on, numbers agreed.

Any pain today, let’s OPQRST,

Onset, provoke, quality expressed.

Radiates or not, zero to ten,

Timing and length, I document then.

Verse 2: HEENT

Eyes first now, sclera bright white,

Conjunctiva clear, pupils equal size.

Light in each eye, direct then consensual,

Nares patent, no drainage or congestion noted well.

Mouth looks smooth, moist, healthy tone,

Ears checked quick, no drainage shown.

Verse 3: Chest & Heart

Chest shape AP to transverse I see,

No lesions front or back on me.

Lungs front, back, axilla too,

Six anterior spots, breathe in deep for you.

Posterior eight, laterals two,

Normal sounds or adventitious I clue.

Heart with bell and diaphragm clear,

Aortic, pulmonic, tricuspid here,

Mitral last, S1 S2 tight,

Rhythm regular sounds just right.

Verse 4: Upper Extremities

Arms are warm, dry, color’s good,

No lesions noted as I should.

Brachial, radial pulses I grade,

Cap refill brisk, strength five displayed.

Verse 5: Abdomen

Supine position, abdomen bare,

Contour observed, skin lesions I scan there.

Auscultate first, thirty seconds or more,

Bowel sounds heard, then palpate core.

Any tenderness, yes or no,

Last bowel movement, color and flow.

Urination changes, pain or blood,

Frequency, urgency, all understood.

Verse 6: Lower Extremities

Legs warm, dry, color okay,

No lesions noted along the way.

Edema check, zero to four,

Pedal pulses I grade once more.

Posterior tibial, dorsalis pedis in line,

Cap refill feet, strength push and pull fine.

Bridge: Focused Finish

Chief complaint deeper, two questions more,

Assess that system beyond the core.

Two extra checks, labs if needed to say,

Head-to-toe complete, check-off slayed.
