Identification

Pharmacology

  • Mechanism of action: Inhibits the 50S ribosomal subunit (bacteriostatic); covers anaerobes and gram-positives.
  • Onset / peak / duration: Rapid; three to four times daily.
  • Half-life / therapeutic level: Short; no routine level.

Clinical use

  • Indications: Skin and soft tissue infections, anaerobic infections, dental infections, aspiration pneumonia, MRSA (some), topical and vaginal for acne and bacterial vaginosis.
  • Usual dose, route, frequency: PO, IV, IM, topical, or vaginal.
  • Maximum dose / adjustments: Caution in hepatic impairment.

Safety

  • Contraindications: Hypersensitivity, prior antibiotic-associated colitis.
  • Black box warning: None (the label carries a prominent warning that it can cause severe and potentially fatal C. difficile colitis; reserve for appropriate infections).
  • Interactions: Neuromuscular blockers (enhanced), erythromycin (antagonism).
