Pharmacology
- Mechanism of action: Same proton pump blockade; available IV.
- Onset / peak / duration: Onset about 1 hour; sustained.
- Half-life / therapeutic level: Half-life about 1 hour with long effect.
Clinical use
- Indications: GERD, erosive esophagitis, hypersecretory states, GI bleeding (IV).
- Usual dose, route, frequency: 40 mg PO daily; IV bolus or infusion for bleeding.
- Maximum dose / adjustments: Reduce in severe hepatic impairment.
Safety
- Contraindications: Hypersensitivity.
- Black box warning: None.
- Interactions: Similar to omeprazole but less clopidogrel interaction.
- Pregnancy / lactation: Use if needed.
- High-alert: No.
Adverse effects
- Common side effects: Headache, diarrhea, nausea.
- Serious effects to report: C. difficile, hypomagnesemia, interstitial nephritis.
- Antidote / reversal: Not applicable.
Nursing process
- Assessment before administration: Symptoms, magnesium with long use.
- Interventions during therapy: Use a dedicated IV line per compatibility; give oral before a meal.
- Monitor: Symptom relief, magnesium, signs of bleeding when used for GI bleed.
- Evaluation / expected outcome: Reduced acid and bleeding control.
